A folder the app was refused is reported as unread, never as empty.
Start with the health check. Its coverage section lists what the app could not read: folders it was refused, folders past the depth limit, pages past the 500 cap, names it could not read, and whether the walk gave up early.
The usual cause is a macOS permission. A folder on an external disk, or in a location macOS protects, needs your permission the first time, and macOS asks once. If that prompt was dismissed, the app is refused every time afterwards without being asked again. Grant access in System Settings › Privacy & Security › Files and Folders.
USB sticks and drives formatted for other systems work. macOS writes its own sidecar files onto them, and the app skips those, so your page count is the real one.
